About Health Management Connect

Health Management Connect works with patients, caregivers, hospitals, physicians and insurance companies to promote and facilitate quality and cost-effective health and wellness for each patient. We serve patients of all ages.

Health Management Connect was formed in 1997 (as Primary Pediatric Management Inc.) to assist families and benefit providers with effective communication and improved informed health care decision making. In 2010, the company expanded to include care for adults and was renamed to Health Management Connect.

Our goal is to assist the patient achieve their optimal wellness so that all parties involved realize a positive outcome.

We are licensed in Missouri, Illinois and subsequent compact states, and to date we have serviced more than 600 families.

Mission Statement

The mission of Health Management Connect is to achieve the wellness of each patient by coordinating the efforts of the patient, the family, the treatment team, and benefit provider while encouraging self-empowerment of the patient and family through advocacy, education, utilization of resources and facilitation of services.
